Assisted Home Care is dedicated to help you transition back to health in the comfort and security of your home. Home health care provides nursing or rehabilitation therapy (physical therapy, occupational therapy or speech therapy) in your home. Your physician determines if home nursing or home therapy is right for you. Home health typically follows a minimum 3-day hospital stay. Nursing at home might also be prescribed if there is a recent change in your health status, or sudden worsening of an existing condition.
Your physician will prescribe what services aremedically necessary depending upon your diagnosis.

After your physician contacts Assisted Home Care to arrange for your home health needs, a member of our nursing or physical therapy team will call you to schedule your first visit. If you have been hospitalized, the first visit is usually the day after discharge, unless otherwise medically indicated.
Following the evaluation, the nurse or therapist and your doctor will develop a plan of care which includes the frequency of visits. Your input and participation are an important part of developing this plan.
Visits are scheduled allowing for some flexibility regarding the time you can expect the nurse or therapist to arrive at your home. Time flexibility allows us to provide each patient with individualized care.
Care is short-term and is not intended to be custodial. Medicare or private insurance must deem the need for home health care to be restorative or rehabilitative. Medicare also requires you to be homebound to qualify for home health care. Assisted will
pre-authorize all your care and verify all your insurance benefits prior to service.
